Understanding fluid from the ear (otorrhea)

Fluid in the ear can be caused by inflammation or congestion that blocks the Eustachian tube — a small drainage passage in your ear. This can happen with an ear infection, allergies or the common cold.

How otorrhea is diagnosed

  • Checking your vital signs for fever
  • Inspecting the ear canal for drainage, infection or a ruptured eardrum
  • Feeling around the ear, jaw and neck for swelling
  • Examining the skin around the ear for redness or inflammation

Tests that may help

  • A physical assessment is usually enough to diagnose otorrhea
  • Hearing tests (audiometry) if hearing is affected
  • Tympanometry to assess middle-ear function
  • A swab or culture if there is persistent discharge
Treatment

Treatment for fluid from the ear

Treatment depends on the underlying cause. Infections are treated with antibiotics or eardrops, while allergies may be managed with antihistamines and decongestants to help the Eustachian tube drain.

If fluid persists despite treatment, a minor procedure such as myringotomy with grommet (ear tube) insertion may be advised to drain the fluid and restore hearing. Keeping the ear dry during recovery is important for a good outcome.
