Understanding ear pain (otalgia)

Ear pain (otalgia) most often happens in children, but it can also affect adults. Pain that starts inside the ear is called primary otalgia, while pain that begins outside the ear is called secondary otalgia.

Whether it is a sharp sting, a dull ache or a burning sensation, ear pain may be constant or come and go, and can affect one or both ears. Studies show that by their third birthday, about 80% of children will have had a middle-ear infection.

Causes

What can cause it

Ear infections

  • Outer-ear infection from swimming, hearing aids or headphones, or cotton swabs
  • Injured or irritated skin in the ear canal that allows bacteria to grow
  • Middle-ear infection following a respiratory tract infection
  • Fluid build-up behind the eardrum
  • Labyrinthitis — an inner-ear infection from viral or bacterial illness

Other common causes

  • Pressure changes, such as flying on a plane
  • Earwax build-up
  • A foreign object in the ear
  • Strep throat or a sinus infection
  • Shampoo or water trapped in the ear
  • Use of cotton swabs in the ear

Less common causes

  • Temporomandibular joint (TMJ) syndrome
  • Perforated eardrum
  • Arthritis affecting the jaw
  • Infected or impacted tooth
  • Eczema in the ear canal
  • Trigeminal neuralgia
Treatment

Medical treatment for ear pain

If you have an ear infection, your doctor may recommend oral antibiotics, eardrops, or both. It is important to complete the entire course even after symptoms improve, so the infection clears up completely.

If earwax build-up is the cause, you may be given wax-softening eardrops, or the wax may be removed by ear lavage (irrigation) or gentle suction. Causes such as TMJ disorders and sinus infections are treated directly to relieve the referred ear pain.

  • Apply a cool washcloth to the ear
  • Keep the ear dry
  • Sit upright to relieve ear pressure
  • Use over-the-counter ear drops or pain relievers
  • Chew gum to help ease pressure
  • Feed a baby to help relieve their ear pressure
